We are in the process of putting all of our applications online but currently the only two that are available is for International Membership and USA Individual Membership. All of the other applications must be downloaded, printed and mailed to the GMCI office.  If you do not have access to a printer you may request an information packet and one will be mailed to you.

MEMBERSHIP DUES
Shows the complete schedule of respective membership dues. (Individual, Church or Outreach Ministry)

INDIVIDUAL/PERSONAL MEMBERSHIP APPLICATION
This application is for USA residents only and is for Ordination, General License, Commissioned Minister or Exhorter.

SPONSOR FORM / USA
This form is to be used by those that have a current GMCI member that will sponsor them. If the applicant has a GMCI 
 sponsor no other recommendations are needed but the Sponsor Form must be filled out, signed and mailed to GMCI by
 the sponsoring member.

CHURCH APPLICATION / USA
This application is for churches within the United States only that desire affiliation for spiritual and/or legal covering. Legal 
covering includes tax exemption status under section 501(c)(3)

OUTREACH / EVANGELISTIC MINISTRIES / USA
This application is to be used by those within the United States that desire affiliation for an outreach or evangelistic ministry

INTERNATIONAL MEMBER APPLICATION
This application is to be used by those living outside of the United States. This application is for Individual or Personal credentials only.

INTERNATIONAL CHURCH AFFILIATION
This form to be used by churches outside of the United States that desire to be affiliated with this international body for 
fellowship and apostolic covering.
